MDT SERVICE ENROLLMENT AGREEMENT
RP Learning Academy | Multidisciplinary Team (MDT)
THIS AGREEMENT is made between RP Learning Academy, located in Accra, Ghana (hereinafter referred to as "the Academy"), and the registering Parent/Guardian (hereinafter referred to as "the Client").
1. SCOPE OF SERVICES
The Academy agrees to provide a comprehensive Multidisciplinary Team (MDT) support system for the Child. This Circle of Care model includes a coordinated effort between three distinct specialists:
• Specialized Therapist: Functional and developmental intervention.
• Professional Counselor: Emotional regulation and family resilience support.
• SEN Facilitator: Academic adaptation and learning support.
The Weekly Schedule: The Client is entitled to four (4) scheduled sessions per week across the MDT pillars, conducted via the Academy’s integrated digital platform or onsite as agreed.
2. FINANCIAL TERMS
Monthly Tuition: The total fee for the MDT service is determined by the selected Milestone Package.
Payment Schedule: Fees are due in full on or before the 1st day of each month.
Non-Refundable Policy: Due to the commitment of three separate professional specialists to your child’s schedule, monthly fees are non-refundable once the month has commenced.
3. ATTENDANCE & CANCELLATION
Consistency: The MDT model relies on consistent intervention. The Client agrees to ensure the Child is present and ready for all four weekly sessions.
Notice of Absence: If a session must be rescheduled, a minimum of 24 hours' notice is required.
No-Show Policy: Sessions missed without 24 hours' notice are considered forfeited and will not be made up or refunded.
Technical Preparedness: It is the Client’s responsibility to ensure a stable internet connection and a quiet environment for digital sessions.
4. THE COORDINATION LOG & PROGRESS REVIEWS
Internal Coordination: The Client acknowledges that the MDT specialists meet internally to align strategies. This coordination time is included in the monthly tuition.
Monthly Progress Reports: The Academy will provide one (1) comprehensive written progress report at the end of each 30-day cycle, summarizing data from all three specialists.
Parent Consultations: One 15-minute monthly review meeting between the Admin and the Client is included to discuss long-term goals.
5. TERMINATION OF AGREEMENT
Either party may terminate this agreement by providing fourteen (14) days' written notice prior to the start of the next billing cycle. If notice is not received 14 days before the 1st of the month, the next month’s tuition remains due.
6. CONFIDENTIALITY & DATA PROTECTION
The Academy agrees to keep all medical, psychological, and educational records of the Child strictly confidential. Data will only be shared between the assigned MDT specialists for the sole purpose of the Child’s development.
7. ACCEPTANCE OF TERMS
By checking the acknowledgment box and providing your digital signature below, the Client acknowledges they have read, understood, and agreed to the terms of the RP Learning Academy MDT Service.
